Schedule An Appointment

If you are a NEW PATIENT to the practice, please select "1) New Patient Visit In Person" to schedule your initial visit. Please be sure to submit the NEW PATIENT PACKET at least a day before your initial visit.


If we have already discussed the medical concern and formulated a plan during a prior visit, please schedule a "Follow Up Visit" to review progress and make adjustments to our plan if needed. 
 

Please select "Extended Visit" if:
-You want to discuss lifestyle changes (smoking cessation, physical activity, dietary changes, etc)
-You want to discuss more than 2 concerns/problems
-You want to discuss uncontrolled mental health concerns
-You would like a procedure (such as laceration repair)
-You would like OMT (osteopathic manipulative treatment)
-You think you might need a longer follow up visit


It is important to discuss preventive screenings that you are due for, evaluate risk factors for disease and discuss lifestyle changes to stay healthy. Once per year, please schedule an "Annual Preventive Visit"
