
DPC Membership Information
Patients may choose a month-to-month membership, or pay for the whole year (the last month of the year is free with an annual membership).
A minimum three month trial membership is required to join.
There is a one time registration fee of $75 to cover the work involved with onboarding a new patient.

HOME VISITS:
For those who have difficulty leaving the home, when deemed appropriate by patient and physician, we offer home visits at an additional fee of $50 per home visit to cover cost of travel.

Please contact us for group employee rates
Non-members can elect to pay-per-visit. Our pay-per-visit rates are $300 per visit, up to 60 mins. These non-member visits are meant to evaluate problems such as rash, UTI, cough, sore throat, headache, ear pain, refills for common medications, etc. Please call our office to schedule an appointment. If you would like to submit to your insurance for reimbursement, a superbill can be provided upon request. Please keep in mind that Renew Family Medicine is considered an out-of-network practice with most insurance plans.
