DPC Membership Information
Patients may choose a month-to-month membership, or pay for the whole year (the last month of the year is free with an annual membership). A minimum three month trial membership is required to join. There is a one time registration fee of $75 to cover the work involved with onboarding a new patient.

HOME VISITS:
For those who have difficulty leaving the home, when deemed appropriate by patient and physician, we offer home visits at an additional fee of $50 per home visit to cover cost of travel.

Contact us for group employee rates
Non-member immediate care visits are available for $200 per 30 mins to evaluate problems such as one-time refills for common medications, rash, UTI, cough, sore throat, headache, ear pain, etc. Please call our office to schedule an appointment. If you would like to submit to your insurance for reimbursement, a superbill can be provided upon request.
If non-members decide to join our practice within 2 weeks of the immediate care visit, we will apply this fee towards the membership
