DPC Membership Information
Patients may choose a month-to-month membership, or pay for the whole year (the last month of the year is free with an annual membership).
A minimum three month trial membership is required to join.
There is a one time registration fee of $75 to cover the work involved with onboarding a new patient.

HOME VISITS:
For those who have difficulty leaving the home, when deemed appropriate by patient and physician, we offer home visits at an additional fee of $50 per home visit to cover cost of travel.

Our pay-per-visit rate for non-members is $300 per visit, up to 60 mins.
Please call or email our office to schedule an appointment. If you would like to submit to your insurance for reimbursement, a superbill can be provided upon request.
(Please keep in mind that Renew Family Medicine is considered an out-of-network practice with all insurance plans except Cigna, PacificSource, and original Medicare)
